Living Legends Nomination Form

The Living Legends Selection Committee evaluates nominees using the following criteria. Nominees need not meet each individual criterion:

  • Length of membership in OANP.

  • Length of teaching career and/or number of years in practice.

  • Pioneering and/or providing leadership to new programs, specialties or areas of inquiry.

  • Advancing the profession through political, organizational, institutional or academic influence.

  • Excelling in leadership positions of distinction.

  • Service to the profession.

With these points in mind, please submit a nominee for this award and describe why this person should receive a Living Legend Award.
