Scheduling a Clinic Appointment

To make an appointment at the NCNM Clinic, or one of our community clinics, simply call one of the numbers below. Appointments are available Monday through Saturday, as early as 8:00 a.m. and as late as 6:00 p.m.

Clinic Patient Forms

We ask that each new patient fill out a registration form and (for adults) a Personal Health History form. Please keep these with you and bring them to your first appointment. These forms can be downloaded below, or a staff member can email them to you at the time you make an appointment.
